• The approximate workload will be 60-70% outpatient and 30-40% hospital acute and swing/SNF
• The clinic is adjacent to the hospital; 5-Star Rated SNF onsite
• As needed, collaborate with physicians and other healthcare personnel on patient progress, concerns, discharge plans, etc.
