Job Details
- BC Internal or Family Medicine
- Starting upon completion of credentialing to end of August
- Multiple outpatient clinics (Oakland, Berkeley, Oakland, Concord & San Leandro)
- Monday - Friday from 8:00am-5:00pm (no call)
- Average of 5 patients per day
- Manage Chronic disease states of geriatric population
- Must work with care plans within group of: NP, IM Doc, PT, OT, dietician, and other specialties for each patient
- Manage dialysis treatment set up
- Coordinate all care for these patients daily
- EMR is E-Clinical works
- BC IM/FM, CA license, DEA, BLS, Geriatric experience, PACE program or Team based practice experience and Covid Vaccine required
