Providence is seeking a part-time provider with geriatric expertise and demonstrated leadership skills to serve as the Associate Director for the Senior Health Program. In this role you will partner with the Medical Director with development of Age-Friendly strategies and initiatives to ensure the efficient and effective delivery of quality patient care for older adults. In this role you will be part of an interprofessional team with geriatric expertise which includes nurse practitioners, a pharmacist, a physical therapist, and caregivers supporting program management and analytics. The mission of the Senior Health Program is to educate teams, cultivate partnerships, influence strategies, and provide innovative solutions to promote Age-Friendly, compassionate care for older adults and their families. The Senior Health Program led efforts that resulted in Providence being recognized by the Institute for Healthcare Improvement (IHI) as a Pioneer Age-Friendly Healthcare System. Ongoing areas of focus include a robust falls prevention program, a dementia Care Ecosystem in collaboration with UCSF, and a four-week Geriatric Mini-Fellowship to upskill primary care providers across Providence. Priorities for this role: This position will collaborate with Senior Health Program leadership in work with Service Area executives, service line leaders, and clinic leadership teams to inform and drive priorities for Age-Friendly care to improve quality, outcomes, experiences, and decrease the total cost of care for older adults. This position will develop and maintain key relationships with leaders across the continuum of health care services within Providence well as the larger community to continue to grow and evolve innovative Age-Friendly approaches, facilitating greater integration across environments of care.
