Responsibilities

1. Works under the supervision of the Chief, Integrated Pain Management Clinic, to provide advanced and complex multimodal pain management to Warriors in Transition, active duty soldiers, reserve soldiers. Subject matter expert on the development of pain management treatment plans.

The Pain Management Anesthesiologist serves as a subspecialty consultant to other departments, services, and health-care providers for a variety of diseases and conditions marked by acute and chronic pain. Performs comprehensive and problem focused histories and physical examinations. Examines, diagnoses, and treats walk-in and clinic appointment patients with a variety of pain complaints. Performs advanced diagnostic and therapeutic nerve block procedures as appropriate to treat acute and chronic pain. Provides follow-up care on patients previously seen in the clinic. Applies the Medical Fitness and Profiling Standard. Evaluates soldiers for retention and admission in accordance with Chapters 2 and 33 of AR 40-501. The Pain Management Anesthesiologist practices mainly in the Interventional Pain Medicine Clinic, evaluating and treating complex chronic pain syndromes affecting Warriors in Transition, and all eligible beneficiaries. Maintains Basic Life Support (BLS). Interprets and correlates laboratory and diagnostic procedures. Interprets and correlates subjective and objective data to formulate a differential diagnosis and establish a working diagnosis. Formulates treatment plans for pain management patients within the scope of practice and as such is the subject matter expert on the development of pain management treatment plans.

Implements therapeutic interventions for specific conditions when appropriate. Exercises judgment on problems requiring consultations, referrals, or evaluations. Orders, collects, and performs appropriate laboratory or diagnostic procedures and studies including, but not limited to, blood and tissue specimens. Orders and performs diagnostic studies such as, but not limited to, EMGs, electrocardiograms, compartment pressure measurements, and examinations including special x-ray examinations, CT, MRI, ultrasound, and bone scans. Writes or dictates physical profiles, medical evaluation boards, fitness for duty evaluations, or other related documentation appropriately. The Pain Management Anesthesiologist educates patients and caregivers about medications and treatment regimens, the quantity of medications, their complex mechanisms of actions and potentially dangerous side effects. The Pain Management Anesthesiologist must maintain accurate, legible, and complete records for all patients treated, given the complex nature of pain medicine. A frequent component of the treatment regimen for Warriors in Transition with complex, polytrauma, and/or phantom limb pain is interventional injection procedures. These advanced interventional injection techniques are frequently done under fluoroscopic guidance. These procedures include interlaminar, caudal and transforaminal epidural injections, selective neural ablation, nerve blocks and neuroma injections in amputated and traumatized limbs. The Pain Management Anesthesiologist also manages patients who have been severely burned and have had amputations or limb salvages. The therapy for these patients includes various blocks and optimization of highly complex therapeutic regimens.

2. The Pain Management Anesthesiologist also serves as a subject matter expert and consultant to case managers and other representatives. of the many groups caring for Warriors In Transition.

Makes decisions on patient dispositions and assists in transfer or admission of patients.

Screens all consults to ensure they are in compliance with the Clinical Practice Guidelines of the Pain Clinic and are accurately placed with the appropriate specialty providers.

Refers patients with complex or unresolved conditions specialty/subspecialty clinics as applicable. Provides telephone consultation with primary care providers regarding pain management.

Conducts consultations in person and over the phone with physicians, physician assistants, or other health care providers, making recommendations and giving guidance for treatment options and providing links between the Pain Clinic and outlying clinics.

Required to maintain certification and license as an Anesthesiologist. Learns new techniques and methods of patient care.

Supervises and assists in training paraprofessionals during their Pain Clinic rotations. The Pain Management Anesthesiologist must maintain a rigorous continuing medical education curriculum, focused on the areas of pain medicine, physical medicine, orthopedics, neurology, neurosurgery, and psychiatry.

1. The pain Anesthesiologist possesses special knowledge of musculoskeletal diseases and injuries and ability to direct a plan of care to this group of patients without supervision. Formal subspecialty training (i.e. Fellowship) and subspecialty Board Eligibility/Certification within Pain Management is preferred.

2. Recognition as an expert within the specialty field and ability to provide consulting services to those within and external to the agency.

3. Knowledge and skill in advanced diagnostic and treatment procedures and protocols.

4. Knowledge of the pharmacological effects of anesthetic drugs and skill in the conduct of management of anesthesia for a broad range of surgical procedures. Ability to administer general, regional, and topical anesthetics and to detect and react to unfavorable reactions.

5. Knowledge of administration and evaluation of actions of new drugs and the evaluation of new anesthetic techniques.

6. Knowledge and skill to serve as an adjunct faculty member providing training and instruction within the specialty field to a variety of post graduate students, interns, nurses, practitioners, and other health care providers.

Must be Board Certified or Board Eligible

Must be board eligible in Anesthesia

Degree: Doctor of Medicine, Doctor of Osteopathic Medicine, or equivalent from a school in the United States or Canada. This degree must have been accredited by the Council on Medical Education of the American Medical Association; Association of American Medical Colleges; Liaison Committee on Medical Education; Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. A Doctor of Medicine or equivalent degree from a foreign medical school must have provided education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.

Requires licensing to practice medicine and surgery in a state or territory of the US or District of Columbia.

Licensure: Applicants must possess a current, active, full, and unrestricted license or registration as a Physician from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
