Positions in this function are part of an interdisciplinary care team of home based medical care providers to support patients facing serious illness, including their families and care givers. In this role, you will serve as the clinical lead for an interdisciplinary care team, responsible for ensuring the highest quality delivery of medically appropriate care to people with serious illness and their families, including through home-based care. You will be working with advanced practice nurses, registered nurses, social workers and administrative team members to provide and coordinate medical care with the patient's community physicians. This is a flexible, team-based role that creates enormous satisfaction as you impact the care and quality of life of our aging population. All the while, you'll be building meaningful relationships with patients, their families, and providers involved in the patient's care to maximize value for some of the most vulnerable individuals in healthcare.

Primary Responsibilities:
  • Provide oversight of assigned interdisciplinary care team(s) providing medical care for patients in their homes and through virtual care modalities
  • Provide care in the home to a primary panel of patients as lead member of the care team
  • Clinical accountability for clinical outcomes for the patients managed within the interdisciplinary care team pods overseen, including hospital admission reduction, clinically appropriate hospice utilization, and appropriate risk stratification of patients
  • Coordinate care and treatment plans with primary care physicians (and other community providers and specialists) to provide the best care possible
  • Lead and or participate in weekly interdisciplinary care team meetings, daily UM hospitalization reviews and multidisciplinary team meetings as necessary.
  • Perform medical consultations and assessments in person and using telemedicine
  • Ensure delivery of medically appropriate care, as well as care that is whole-person centric to address certain social factors that impact health status
  • In partnership with other physician leaders, develop education plans, prepare curriculum and present on educational topics such as serious illness communication, advanced illness management and symptom management, among others
  • Provide administrative and clinical guidance and support to the care team to ensure quality outcomes for patients and families and ensure clinicians are operating at the top of their licensure
  • Periodically review and approve care team clinical documentation and orders, and provide coaching feedback for areas of clinical competency and accuracy
  • Provide medical oversight for advance practice nurses
  • Be part of an after-hours care team support system for patients to prevent unnecessary or avoidable hospitalizations and ED visits
  • Provide rotational on-call coverage after hours with other regional physicians for care escalation
  • Support performance improvement projects, as identified

Required Qualifications:
  • Valid, unrestricted license to practice medicine in the state
  • Ability to obtain DEA licensure/Prescriptive Authority post-hire in states where applicable
  • Clinical background in adult, family or geriatric specialties with experience managing medically complex patients with serious illness
  • Ability to work within an interdisciplinary care team to coordinate patient care

Preferred Qualifications:
  • Board Eligible or Certified in Family Medicine, Internal Medicine, Geriatrics or Hospice and Palliative Medicine (HPM)
  • Excellent administrative, organizational and verbal skills

Physician - Core Expectations

Weekly Meetings
  • Monday Morning Sign-out Rounds-weekend physician signs out calls received during the weekend with the clinical team and follow-up needed
  • Team meetings- Interdisciplinary team meetings are Monthly to Bimonthly, depending on market, large group meetings that focus on education, clinical policy changes, metrics.
  • Weekly Clinical Meetings when the clinical teams are kept abreast of changes or updates in clinical policy or the documentation platform.
  • Regional meetings- physicians; market, regional, associate medical directors, and collaborating physicians.
  • Local meetings-local team leadership- AMD, Regional Medical Director, Clinical Director, and Regional Vice President.
Daily meetings
  • Hospital huddles-review patients that were hospitalized and discuss follow up care and further hospitalization prevention
  • Patient Care Meetings (PCM)-smaller group meetings- two to four clinicians (each smaller grouping meets weekly) to review in depth the patients for treatment planning, prevent avoidable hospitalizations and identify hospice eligible patients. We also function as a "think tank" to address complicated or concerning patients- area specific
  • Afternoon sign out of clinical team to on call physician and triage team of any after hour patient needs or needed follow up
Call
  • Be part of an after-hours care team support system for patients to prevent unnecessary or avoidable hospitalizations and ED visits
  • Covers regional area with variable rotation based on the coverage area and number of physicians.
  • immediately available to the afterhours nurses for questions or to speak directly with patients
  • Call coverage includes nights, weekends and holidays on a rotation basis
  • Obtain licensure in call coverage states per RMD
Daily responsibilities
  • Provide oversight of assigned interdisciplinary care team(s) providing medical care for patients in their homes and through virtual care modalities by being immediately available during the day to assist RNs, SWs, and NPs telephonically and via video when they are in the field.
  • Examples: recommendations regarding next best steps, medication prescribing, or serious illness discussions.
  • Complete hospice referrals and care coordination with hospice CMO/staff
  • Care coordination with primary providers and specialists
  • Order DME, and home health as appropriate
  • Provides medical oversight for advance practice nurses and collaboration with NPs per state guidelines
  • Perform medical consultations and assessments in person and using telemedicine
  • Review medical records and document summary in EHR
  • Document all patient conversations in EHR
  • Handle urgent calls during the day when nurses are not available
  • Review and approve care team clinical documentation and orders, and provide coaching feedback for areas of clinical competency and accuracy
Leadership
  • Serve as a role model for Prospero Core Values and as a member of interdisciplinary teams where all disciplines are valued equally to deliver person-centered care.

  • Partner with local clinical director and clinical service manager to mentor clinical staff
  • Provide administrative and clinical guidance and support to the care team to ensure quality outcomes for patients and families and ensure clinicians are operating at the top of their licensure
  • Ensure delivery of medically appropriate care, as well as care that is whole-person centric to address certain social factors that impact health status
  • May require occasional travel for training, patient visits or NP/RN clinical oversight

Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
