Cleveland Clinic is currently seeking a Hospitalist to join our rapidly growing Center for Connected Care. Candidates must be Board Certified/ Board Eligible Internal Medicine. We are currently expanding our value-based post-acute programming into the Long-term Acute Care Hospital (LTACH) environment. The Centers goal is to ensure world-class care across the continuum of care by connecting patients to the highest-quality post-acute care programs and facilities as they transition from the hospital back to the community.

This position has the unique benefit of a balanced work/home lifestyle. There will be assignment to one or more community-based LTACH facility locations. The Hospitalists schedule is days only and will work 15-20, 8-10 hour shifts per month (including weekend responsibilities of 3-4 days a month). Flex hours are available. Providers in this innovative program will enjoy the support of a multidisciplinary specialty team of Cleveland Clinic professional staff. Opportunities for Leadership and Medical Direction also exist with this position.
