- This is a multi-site FQHC serving 80% of the primary care needs in our community
- Our mission is to serveour patients with quality, efficiency, and accessibility
- As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state, and regional health care environment
- Our Health Center sites are certified Level111 Medical Homes, with a highly developed and supportive panel management and case management system in place
- Robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners, and Licensed Clinical Social Workers embedded within its primary care sites
- We have a fully integrated electronic medical record and practice management system andinterface abilitywith our local community hospital
- After-hours call (triage only) is shared among sites. Call responsibility is minimal.
- Responsibilities include empowering patients to take an active role in health maintenance and promote wellness behaviors, providing appropriate patient follow-up and communicating effectively with patients, referring patients to specialty care as necessary, providing clinical supervision to mid-level providers and nursing staff, and participating in quality assurance activities
