NYC Health + Hospitals/Harlem, located on 135th Street in Manhattan, is a designated Level 1 Trauma Center and is recognized for providing centers of excellence in the areas of Bariatric Surgery, Breast Imaging and Sexual Assault Forensics. Our teams provide services for nearly 213,000 outpatient visits, more than 83,000 emergency room visits and over 13,000 inpatient admissions annually. The hospital maintains an academic affiliation with Columbia University Medical Center.

Responsibilities:

  • To support the New Opportunity for Women (NOW) Program clients receive coordinated medical case management services to navigate the health care system and psychosocial barriers to optimal health and family stability.
  • Conducts comprehensive program Intake/assessment, reassessment, service plan development, case conference with medical and non-medical providers at least semiannually. Administers annual program mental health assessment that is followed by PHQ 9 every 6 months thereafter.
  • Monitors clients adherence to medical and specialty appointments of index patient and exposed children per clients service plan and or provider recommendation and at monthly checking calls.
  • Monitors and track patient's viral load and retention in care status and provide support, education and intervention/including appointment reminder, missed appointment follow up and advocate with social service agency and community based organization to ensure all clients needs are met.
  • Monitors and provide support to all HIV positive women who are pregnant regardless of program enrollment and ensure retention in care, Viral load and STI screenings are being conducted throughout the pregnancy, especially in the 3rd trimester
    Responsible to participate and present cases at case conferences, program and department QI/PI meetings
  • Completes daily progress notes and daily logs for services rendered on a day to day basis and submits by COB each day.


Qualifications:

  • License Master of Social Work Degree from the state of New York
  • Direct care/case management experience working with the HIV population
  • Must be bilingual (English/Spanish)
  • Proof of Covid 19 vaccination required prior to hire
