Providence Connections Palliative Care is seeking a full-time Palliative physician to join its established interdisciplinary team in Portland, Oregon. In this role, you'll be providing consult services in Portland hospitals throughout the area, working closely with nurses, social workers, and chaplains caring for seriously ill patients. Successful candidates will have completed a Hospice and Palliative Care fellowship program and be board-certified or eligible through AAHPM. Connections Palliative Care is part of a growing, integrated Home and Community Care program which collaborates with multiple specialties as well as community-based practitioners to care for patients and their families.
Where You'll Work
Providence Connections is a Palliative Care consultation service for patients living with a serious illness. This interdisciplinary group helps patients and their families find needed resources to control symptoms associated with their illness and maximize quality of life, all while providing compassionate support. Teams of physicians, nurse practitioners, Palliative Care nurses, clinical social workers and chaplains work collaboratively with all members of patients' care teams to provide holistic care.
