Are you a Physician ready to be a part of a healthcare system different from what you've seen before? Carbon Health is looking for passionate Primary Care Physicians to join our remote, and tight-knit team working together to continue to push the boundaries of what excellent virtual care is.

Primarily through video visits and some messaging, you'll deliver care to patients from multiple states (Great if you already have multiple state licenses. If not, we will work with you to get them!). With our own custom EHR built around our care delivery, and streamlined communication, you'll get to spend more time with patients, and less time on administrative work. What You'll Do:

• Manage a panel of patients towards evidence-based clinical quality targets 
• Work collaboratively with the extended care team to follow up on patient results
• Supervise and teach advanced practice clinicians
• Provide input and implement care programs, pathways, and new product enhancements
• Collaborate with the behavioral health team to diagnose and treat patients’ mental health
• Collaborate with Carbon’s omnichannel care services (i.e., Diabetes Management, Hypertension, Remote Patient Monitoring, Metabolic Health)
• Provide excellent preventive, proactive, compassionate, evidence-based primary care via virtual visits
• Participate in an after-hours on call rotation schedule with other clinicians

About You:

• Multi licensed MD/DO board certified in Family or Internal Medicine with an unrestricted DEA license
• Unrestricted active Massachusetts license
• Additional active licenses in the following states preferred:
  • CA, CT, FL, ME, NC, NH, NJ, NY, PA, RI, TX, VT
• Must be eligible and willing to obtain medical licenses in all 50 states and the District of Columbia
• 3+ Years of clinical practice post-residency
• Clinical proficiency in evidence-based primary care with experience in longitudinal care
• Experience working as a telemedicine physician preferred
• Skilled at using online tools and technology to deliver care and communicate with patients
• Passionate about relationship-driven, human-centered primary care
• Experience working in a value based care environment
