Job Details
- Specialties: IM, FM, GER, CARDIO, N, END, PUL, NEP, ID, HEM, RHEUM, EM, PM&R
- Board certification in specialty/area of practice that is recognized by the American Board of Medical Specialties (ABMS)
- Hours: 8 am - 7 pm Mon- Friday and Saturday/Sunday 10am -6pm
- Required Licensure: NY
- Required Experience: 2+ years practicing experience as a licensed Physician following completion of a postgraduate medical education residency program
- Required Skills: Computer literacy with ability to navigate technology and perform telehealth assessments; highly developed written and oral communication skills; strong interpersonal skills; ability to work in a stressful and changing environment; and proven ability to work with community groups.
- Strong preference for someone that can speak Spanish, Russian, Haitian-Creole, Chinese, Korean or Bengali
- This position supports the statewide Independent Practitioner Panel that issues Practitioner Orders for individuals who have undergone a Community Health Assessment and are eligible for community based long term care services and supports. The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment system.
