Responsibilities:
Primary Function:
The person hired for this position with assist in the planning, executing, and clean-up of regularly scheduled hospital- and community-based HIV testing events. Tasks will include:
- Identification of community health fairs, event promotion, ordering of materials, booking event space and coordinating with event space staff.
- Interact with clients to perform & document rapid, point-of-care HIV testing in accordance with hospital, local and state requirements and guidelines.
- Provide clients receiving rapid, point-of-care tests appropriate pre- and post- counseling and education. Responsible for linking newly diagnosed HIV+ to care in the Jacobi or North Central Bronx HIV clinics for immediate antiviral treatment.
- Responsible for linking eligible patients to the appropriate clinical outpatient care. This includes clients with newly diagnosed HIV+, out of care HIV+, sexually transmitted diseases, and clients interested in PrEP and/or PEP.
- Provide counseling & PrEP/PEP education for HIV- clients.
- Responsible for PrEP 6 month's re-assessment of patients enrolled into the PrEP program.
- Responsible for all EMR data entry, partner notification and quality improvement efforts.
- Coordinate patient referrals to CBO's for continued counseling of high risk patients.
- Participation in focus group and individual interviewing of patients for program enhancement.
- Attend Job related education training/seminaries.
- Performs miscellaneous job-related duties as assigned.
- Support the Program Manager and the Clinical Care Coordinator as needed to provide support to our HIV- and HIV+ clients.
Organizational Relationships:
Reports to the Program Manager in regard to HIV testing events and to the Program Director for clinical issues.
Works collaboratively with other Patient Navigators, Clinical Care Coordinator, Follow-up office, hospital staff, volunteers & interns affiliated with the PS21-2102 Grant.
Qualifications:
Minimum of a College Degree is required
Preferred:
Cultural competence with the LGBTQ community
English and Spanish skills
Identification with BIPOC
Proof of Covid-19 vaccination required prior to hire
