Permanent Virtual Home Hospitalist Physician Job in Worcester, MA - UMass Memorial

UMass Memorial Health, the health and wellness partner of the people of Central Massachusetts, the largest health care system in Central Massachusetts and the clinical partner of UMass Chan Medical School, seeks a Virtual Home Hospitalist to join the growing Hospital at Home (HAH) program. Our HAH program is an innovative program that provides high-quality, hospital-level care in the comfort and familiarity of the patient's home. Studies show that HAH programs improve patient outcomes and increase patient satisfaction.

The Virtual Home Hospitalist closely collaborates with the UMMH HAH nursing and community paramedic caregivers to provide telehealth facilitated inpatient level care in the home for appropriately selected patients requiring ongoing inpatient level acute care. On a routine basis, the Virtual Home Hospitalist

• provides clinical consultation and support to the HAH Care Navigator RNs screening patients for admission to the UMMH HAH program,
• facilitates admissions to the UMMH HAH program,
• serves as the primary responding clinician and caregiver for patients admitted to the UMMH HAH service, and
• models the highest standards of care for an often social and medically vulnerable population.

The successful candidate will be flexible, self-assured, have acute care hospital medicine experience in the past five years, a growth mentality, aptitude for learning, and comfort with technology. BE/BC Internal Medicine or Family Medicine required.
