Description

Join our growing Palliative Care team in Southwestern Connecticut. You will join a well-established and respected inpatient palliative care team and a growing outpatient presence. Our inpatient team provides consultative services at a 501 bed academic community hospital with two campuses. The outpatient position is expected to grow to a five days per week service to support our oncology patients with a full outpatient interdisciplinary team including physician, RN care coordinator, social worker, and chaplain. There are many opportunities for clinical education and research, and the option exists for a voluntary clinical faculty appointment with Yale. The Section of Palliative Medicine works in close collaboration with a large Section of Geriatric Medicine and provides education to residents, fellows, nursing students, and social work students from many surrounding institutions including Yale. This team was recognized for the 2018 Empathy Amplified Award from the Cleveland Clinic for innovative programs and highly empathetic patient/family centered care
We are seeking a Board Certified/Board Eligible internal medicine or family medicine physician who has completed fellowship training in Hospice and Palliative Medicine.
