General Summary

The Medical Director, Population Health will provide strategic clinical oversight and process improvements for population health programs at KPWA. They will collaborate with the WPMG Director of Population Health Programs and the KFHP manager for outreach and screening. This position supervises the Clinical Lead, Condition Management for management of the populations within their portfolio. They directly manage population health programs addressing well care, immunizations, routine screening programs for cancer and infectious diseases, mental health and wellness, and cardiovascular health. The position is responsible for achieving performance and outcome targets for optimal delivery of quality care for the entire population health portfolio which includes WPMG and Network populations, and will work collaboratively with KPWA, WPMG and Network Medical Directors to achieve these outcomes.

Principal Duties and Responsibilities
  • Collaborate with the Medical Director for Clinical Knowledge and Implementation on Evidence based guidelines or best practices to guide population health programs.
  • Responsible for the clinical definitions of patient populations based on clinical evidence and guidelines in accordance with appropriate regulatory bodies.
  • Partner with Director, Population Health Programs to develop and implement automated processes for outreach, screening, and management of preventative care and screening programs.
  • Supervise the Clinical Lead, Condition Management to oversee population health care for members within their assigned portfolio of conditions.
  • Collaborate with operational care delivery counterparts to institute practice models and workflows that are in support of optimal population health care, including prevention and screening as well as condition management.
  • Collaborate with the Medical Director for Clinical Knowledge and Education on Patient and provider education to support population health care.
  • Collaborate with the Director Clinical Knowledge, the medical informatics team, and care delivery leaders on clinical tools, reporting, and patient activation tools to support optimal processes population health care to occur smoothly in distributed environments where clinically appropriate and/or technically necessary.
  • Develop and improve measurement and reporting strategies to measure processes and outcomes. Use these tools to provide feedback and continuous improvement opportunities to population health teams and Care Delivery teams.
  • Annually achieve or exceed regional targets for population health care as set forth by KPWA quality leadership, for each sub-population individually and for the overall population.
  • Incorporate an equity lens in all activities, including (but not limited to) evaluations of systems, processes, and people and program development.
  • Provide medical direction over development and implementation of automated processes to optimize prevention, screening, and safety net programs for commonly missed diagnostic/screening opportunities.
  • Supervise and lead quality champion content experts for the prevention, screening, and safety net programs within scope (including but not limited to Wellness/immunizations, cancer screening, mental health, and cardiovascular care).

  • Knowledge of delivery system operations, healthcare delivery systems and trends.

  • Knowledge of business process and rapid cycle improvement and systems thinking. Knowledgeable about care delivery across the continuum. Expertise with Microsoft Office Suite

Qualifications

Experience

WPMG Shareholder status \nClinical experience in healthcare setting: Experience in a highly collaborative and/or matrixed environment.\n
Five years of medical practice.
Experience in medical staff leadership position that included one or more of the following: performance management of direct reports, budget responsibility, working in and/or leading a multi-disciplinary leadership team, working in a managed care environment. \n
Preventative Medicine Residency certification
Board Certified
Credentialed by KPWA
Unrestricted Washington State medical license.
MPH or other additional training in population-based medicine
Completion of approved residency program or certification.
MD, DO or PA-C with experience in a health care business leadership role.\n
