Seeking a physician to provide clinical expertise within the Supportive Medicine team to improve quality of life and health for patients with complex chronic illnesses and/or approaching end-of-life illnesses. The candidate will b uild effective relationships with referring physicians to ensure excellent team follow-up regarding patient consults and to build an improved culture of collaboration among specialists, primary care physicians, nurses, and other staff to meet the needs of Supportive Medicine patients; proactively identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resources; and ensure compliance with legal, regulatory, and clinical policies, and procedures.

Responsibilities include:
  • Provide consultation services in Geriatrics and Palliative Care
  • Provide education/guidance regarding the scope of Supportive Medicine/Palliative Care services to group physicians
  • Monitor and supervise Supportive Medicine home care delivery team(s)
  • Facilitate clarification of patient and family goals of care
  • Diagnosis, treatment, and management of acute and chronic healthconditions
  • Order, perform and interpret laboratory and radiology tests
  • Prescribe medications including controlled substances
  • Order treatments and durable medical equipment as indicated
  • Perform other therapeutic or corrective measures as indicated, including urgent care
  • Consult with primary care physician of services provided and collaborate with other physicians as needed
  • Assist in all facets of care coordination for palliative care referrals
  • Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
  • Assist in the development of clinical practice guidelines/standards in support of quality palliative care
  • Timely review, oversight, and documentation of patient care by team NP (s), RN, LVN, and other licensed or non-certified team members
  • Education of staff
  • Adhere to the practice of confidentiality regarding patients, families, staff, and the organization
  • Home and Nursing home visits as needed for patient care continuity.
  • Work with the administrative team to improve the operational efficiency of the Supportive Medicine section


Qualifications include:
  • Doctor of Medicine
  • Board Certification in Family Medicine or Internal Medicine
  • Board-certified or Board Eligible in Geriatrics and/or Palliative/Hospice Medicine preferred.
  • Current Texas medical license
