Cleveland Clinic Florida announces a search for a Regional Institute Chair for the Population Health Institute. Goals for this leadership position will include development of clinical programs in the area of population health care and clinical integrated networks within the Cleveland Clinic Florida region. The role will be responsible for oversight and management of clinical outcomes, quality and patient safety, patient experience, caregiver engagement, strategic programmatic growth, physician development and research and education. In addition, the Institute Chair will manage the team responsible for the Cleveland Clinic Florida's value based care strategy, which includes our clinically integrated network (Cleveland Clinic Florida Integrated Health) and our Cleveland Clinic Florida Accountable Care Organization.

The Regional Institute Chair for Population Health will include the following Departments and Sections: Concierge Medicine, Executive Health, Express Care, Family Medicine, Geriatrics, Internal Medicine, and the Value Based Care Team.

The Population Health Institute provides care to 17 locations throughout five counties, providing comprehensive primary care and community care services. The Center includes 135 Staff physicians and 51 advanced practice providers from the Indian River, Martin and Weston employed medical groups.

The Population Health Institute will partner closely with other regional centers and teams to coordinate primary care and performance in value-based contracts consistently throughout all of our Florida locations. This also allows us to enhance the patient experience and align our specialty departments to work more closely with our colleagues throughout the Florida region to improve patient care and outcomes.

In addition to the formal reporting elements listed above, the person assuming this role will be a strategic

thought partner for Regional Leadership about how we accomplish the following:

1) Design and equip our teams for success in value based care arrangements

2) Partner with our payer contracting teams to ensure that risk assumptions stay in lock step with our

operational and care delivery capabilities

3) Work with operations and finance to ensure that work flows and measures of success adapt as we

transform how we care and get paid

4) Inform regional strategies around alignment opportunities with independent physicians

5) Partner with community based organizations in promoting health equity

6) Be an internal and external voice for population health efforts in the region

The selected candidate will work with Population Health Center Directors and the Hospital Executive Teams on development of referrals within the network and defining quality value based metrics. He/She will also need to define best practices and integration of the service lines between hospitals in the region. This leader will work with local, regional, and enterprise stakeholders to ensure that best practices from across the Cleveland Clinic Florida region are incorporated.
