Cleveland Clinic is seeking board certified/ eligible physicians to join our Department of Internal Medicine at our Lakewood Family Health Center. A successful applicant will have special interest and/or experience in healthcare for sexual and gender minority communities, HIV care/prevention, and transgender healthcare. Care offers healthcare services at the Lakewood Family Health Center alongside the Transgender Surgery and Medicine Program (TSMP).

Lakewood Family Health Center is also an upcoming academic hub for medical students and Internal Medicine residents. Physicians work within an outpatient setting, providing adult primary care and consultative services for patients with complex clinical problems, with an emphasis on teaching, leadership and scholarly activity. Embedded in a primary care practice, the center provides care for all patients in a safe and welcoming environment. It includes providers who understand the health needs of LGBTQ+ patients, and access to specialists with expertise in LGBTQ+ care.

Our practices have achieved medical home certification, use Epic™ electronic health record, and are focused on quality improvement, population health and innovation in care delivery. Team based care with embedded Pharm Ds, Behavioral Health Social Workers, telephonic Primary Care Social Work, RN Care Coordinators, Geriatricians and growing Advanced Practitioner partnerships. We are building out systems to innovate flex time within schedules to focus on enhancing value based care to maximize quality and efficiency with reduced costs while delivering exceptional patient and team experience. Educational innovation is also prioritized.
