Per diem Internal,Family or Geriatric Medicine Opportunity in Connecticut

Hartford Healthcare Medical Group (HHCMG) is seeking a boardcertified/board eligible internal medicine, family medicine or geriatricstrained physician to join our growing Geriatrics program. Built on an institutemodel, our program currently provides Geriatric care, consultation andexpertise to geriatric patients in various ambulatory and facility settings.

In this role, we are seeking a physician to provide roundingin Skilled Nursing Facilities on a per diem basis the central Connecticut area.

Position details:

• Per diem, as needed and mutually agreeable schedule
  
• Location(s) negotiable
  

Requirements:

• Passion for providing patient-centered care to ageriatric patient population focused on quality, prevention and education
  
• A willingness to work closely andcollaboratively within an interdisciplinary team
  
• An unrestricted State of Connecticut MedicalLicense (or eligible)
  
• Board Certified/Board Eligible Internal, Familyor Geriatric T Medicine Physician

Qualifications
Licensure / Certification / Registration:
• Required Credential(s):
• Licensed Physician MD/DO obtained prior to hire date or job transfer date.

Education:
• Doctor of Medicine required
