The Division of Geriatrics in the Department of Medicine at Duke University seeks a full-time clinician-educator to meet a growing demand for geriatrics expertise across the health system. This includes providing and coordinating care for older adults with multiple medical conditions, functional decline and psychosocial complexity. This recruit that will partner with division leadership and other faculty as we expand innovative models of care, including HOPE (a transitional care model) and POSH (peri-operative optimization of senior health) programs as we forge the expansion of geriatrics to establish a new evidence based clinical delivery model at Duke Raleigh Hospital.

The ideal candidate would see patients in a mix of settings, including the Geriatric Evaluation and Treatment (GET) clinic, but focus on Duke University Health System Hospitals to provide consultative, peri-operative management (POSH), and transitional (HOPE) care. We are looking for a faculty member interested in developing clinical processes and efficiency, deepening inter-professional collaborations, and establish high quality value-based clinical programs that maximize geriatrics expertise in our health system. Academic engagement will entail clinical program development, inter-professional clinical education and quality improvement initiatives to facilitate promotion and career advancement. This includes participation in education of health professionals and trainees through the newly formed Inter-professional Education Center led by a senior geriatrics faculty member.

The successful candidate is board certified in internal medicine and fellowship trained in geriatrics and committed to education of geriatrics fellows, internal medicine residents, medical students, nurses, therapists, advance practice providers and the community.
