- Fantastic opportunity to work with a medically complex GERIATRIC patient population in an integrated outpatient setting

- The care team includes Health Coaches, nurses, nurse practitioners, and behavioral health specialists
- Lead systemic change by building relationships with outside medical consultants, hospitalists, ER providers, etc?
