DEPARTMENT: Geisinger at Home

LOCATION: Snyder, Union, Northumberland and Lycoming counties

WORK SCHEDULE:Days/Evenings/Call Rotation

WORK TYPE: Full Time

Geisinger at Home is a portfolio of services designed to provide integrated clinical care that manages patients with multi-morbid medical conditions primarily within their homes by delivering:

• Comprehensive medical care in the home

• Community based palliative care and coordinated handoffs to hospice

• Acute care by mobile paramedics

The program is designed to deliver a spectrum of clinical services that help high-cost, complex patients with care at home to avoid tertiary settings when possible.

Geisinger at Home is seeking a Regional Medical Director for the central region. The Regional Medical Director will provide regional leadership in coordination with the Chief Medical Officer of Geisinger at Home. Provide direct care and supervise advanced practitioners in the Central region while working collaboratively with the Chief Physician Assistant. Implement, coordinate and evaluate strategic initiatives focused on home based interventions which include, but not limited to, clinical evaluations, administration of intravenous medications and other in-home treatment options for managing acute presentations in conjunction with direct case managers and other HBMC staff. A predefined population will include medically complex, often frail patients. Works closely with key stake holders across the Clinical Enterprise regionally with an emphasis on value based care. Familiarity with managing clinical utilization and reviewing financial data to help guide clinical optimization plans.

Maintains overall fiscal responsibility for the regional program and performs duties to support the goals and objectives of the program. Responsible for the analysis, development and implementation related to programmatic growth and expansion of Home Based services.

EDUCATION AND/OR EXPERIENCE:

MD or DO required. Experience managing complex patients within a hospital setting or facility preferred.

Minimum 3 years clinical experience required preferably with acute care or health care system background. Familiarity with ACO or value based care ideal but not required.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Work is typically performed in an office environment and patients' homes. Involves occupational exposure to disease illness including, but not limited to, HBV and HIV viruses. Frequently stands, walks, sits climbs stairs, lift/carries up to five pounds, and drives an automobile.
