  • Intermountain Healthcare is looking for one BC Family Medicine or Internal Medicine physician to provide Post-Acute Care Services for Homecare
  • As part of the Post-Acute Care Team (PACT), physician will provide initial assessment and ongoing care of patients discharged from Intermountain hospitals to skilled nursing facilities (SNF). The goal of this service is to enhance the quality of care and assure seamless transitions of care for this patient group
  • Provide subsequent visits in SNF's for PACT patients based on need
  • Collaborates, educates, and supervises Advance Practice Providers in managing acute and long-term medical needs of patients
  • Eight hours a week (.2 FTE)
