Geisinger Community Medicine consists of 43 community practice sites throughout central, south-central and northeast Pennsylvania. These multispecialty locations provide primary and specialty care. In conjunction with Geisinger Health Plan, the Medical Groups staff offers their own unique version of medical home.

Geisinger Lycoming is seeking a board certified/board eligible primary care physician to join our outpatient primary care clinic in Montoursville, Pa. The staff at Geisinger Lycoming is committed to providing the best care for every patient and as experts in primary care, meeting patients' needs is our top priority.

Geisinger Primary Care Model:

• Appointments: Twenty minute appointments for routine visits and forty minute appointments for our elderly patient population (age 65+)
• Daily huddles: With all care team members to review essential information about schedules, patients, resource needs and coordination of care.
• Patient access to care via: eVisits, patient portals and Geisinger's Careworks urgent care clinics.
• Support staff includes: Nursing, clinical and clerical staff as well as pharmacists, care managers, health managers, clinic health associates and coding educators
• Electronic health record: Epic - fully integrated
• Teaching opportunities: Geisinger has a long-standing commitment to medical education and embraces its responsibility in training the next generation of physicians, advanced practitioners, nurses and other healthcare professionals.
• Fresh Food Farmacy™: Patients who are identified as having HBA1C levels greater than 8 and as being food insecure are given a "prescription" or referral by their primary care physician for the Fresh Food Farmacy. The Fresh Food Farmacy is an example of the Geisinger Medical Home model which brings team members together, working collaboratively to ensure the best outcomes for patients.
• Research & genomics: At Geisinger, we've decided to be more precise on what specific conditions our patients may face in the future. By proactively looking at their genetic code as part of their routine care, we can lower their risk, treat at early stages or help them avoid certain conditions before they even take hold.
