JOB TITLE: Regional Medical Director, Geisinger at HomeLOCATION: Lewistown, PAWORK SCHEDULE: Days/Evenings/Rotation/On CallWORK TYPE: Full-time Geisinger, a national leader in healthcare innovation and technology, is seeking a BC/BE Family Medicine, Internal Medicine or Emergency Medicine physician for an exciting opportunity with the Geisinger at Home team, based in Lewistown, PA. Geisinger at Home is a portfolio of services designed to provide integrated clinical care that manages patients with multi-morbid medical conditions primarily within their homes by delivering Home Based Medical Care (HBMC). The program is designed to deliver a spectrum of clinical services that help high-cost, complex patients with care at home to avoid tertiary settings when possible. Home Based Medical Care team includes:Advanced Practitioner- Direct Care Case Manager
- Direct Care Team Community Health Assistants
- Field Coordinator
- Physician (Regional Medical Director)
The Regional Medical Director, Geisinger at Home, will provide regional leadership, in coordination with the Chief Medical Officer of Geisinger at Home, direct care and supervise advanced practitioners in the Northeast region while also working collaboratively with the Chief CRNP. Implement, coordinate and evaluate strategic initiatives focused on home based interventions which include, but not limited to: Clinical evaluations, administration of intravenous medications and other in-home treatment options for managing acute presentations in conjunction with direct case managers and other HBMC staff. Responsible for the analysis, development and implementation related to programmatic growth and expansion of Home Based services. Regional Medical Director FTE breakdown is .8 FTE Geisinger at Home, .2 FTE primary clinical discipline within the Medicine Institute. A minimum of 3 years clinical experience required, preferably with acute care or health care system background. Familiarity with ACO or value based care ideal but not required.
