One of the ten “pillars of care” of Miami Cancer Institute is palliative care. Palliative care has been elevated to its own pillar because of its increasing role in the overall care of the patient with cancer.

Palliative care physicians, empowered by the treating medical or radiation or surgical oncologist from the very outset of diagnosis as an essential part of the multi-disciplinary team, perform two key functions. Number one, they are another provider who can effectively deal with a patient’s symptoms, especially in the areas of pain, anxiety, depression, constipation, or nausea/vomiting. Number two, and perhaps most essential, they have the time to meet with the patient, often together with the patient’s surrogate as well, over several visits, in a relaxed, less “clinical” environment, and discuss a patient’s understanding of their disease, prognosis, treatment options, and options if and when treatment options are exhausted. This leads to more shared decision making, and hopefully to better, less stressful, more personalized, and more patient centric decisions at the end of life, often accompanied by an “official” advance care plan.

We are proposing to hire a lead physician to head an outpatient oncology palliative care program at Miami Cancer Institute. This person will organize the program, be responsible for establishing liaison with the treating oncologists, establish smooth transitions between the outpatient and inpatient setting (where the inpatient palliative care program resides), recruit and train advanced practice providers to extend such care to many more patients, ultimately recruit an additional physician or physicians as needed, and conduct clinical research examining the benefits of such a program. She or he will also do palliative care visits themselves, educate the community and potential donors about the program, and assist marketing to increase awareness of this unique program.
