PALLIATIVE CARE PHYSICIAN

OUTPATIENT ONCOLOGY

Fairfield County - Connecticut

The Palliative Care Physician will lead our outpatient oncology efforts at Yale New Haven Health—Smilow/Bridgeport Hospital Campus. This is an opportunity to build an oncology focused practice, with leadership and growth opportunities within the Yale New Haven Health System, all at our new state of the art outpatient facility. As a part of Yale New Haven Health, Bridgeport Hospital offers cancer care and treatment at Smilow Cancer Hospital Care Centers in Fairfield and Park Avenue Medical Center in Trumbull. Cancer patients who are admitted to Bridgeport Hospital are cared for by specially trained staff on a dedicated oncology unit.

The practice will involve expanding relationships with referring staff, development of oncology specific expertise, education at all levels (GME, Medical and Nursing Staff, Community), and operations related to the clinic experience. YNHH—Bridgeport Hospital Campus offers a wide range of patient populations, and outstanding clinical and academic relationships with the Yale New Haven Health main campus and Yale University. Opportunities for scholarly work and collaboration across campuses exist. Position will be primarily outpatient, but remain integrated with the inpatient hospital team for reciprocal coverage and support. Inpatient team is well established (7 years), fully interdisciplinary, and has a warm and supportive culture. The position will report to the Medical Director of Palliative Care, Bridgeport Hospital, and reside within the Center for Geriatrics and Palliative Care, offering additional collaborative benefits with Geriatrics. Bridgeport and surrounding areas offers wonderful living possibilities with easy access to New Haven and New York City.

Responsibilities
  • The Palliative Care physician provides health assessment and direct care in all our practice environments to patients as part of the provider team. The Palliative Care physician provides comprehensive medical services for patients, regardless of age or sex on a continuing basis by performing the following duties outlined in this document. The medical director of Pain and Palliative Care service identifies and problem solves issues that arise with Pain and Palliative Care Services. (S)he provides quality monitoring and performance support for all Pain and Palliative Care Services advanced clinical providers. Oversees co-ordination of education at the level of fellowship, PA and Medical student, and APRN student. (S)he assists in the implementation of strategy to grow Pain and Palliative Care Services. (S)he oversees the implementation and operation of PalGEMS, which is the ED- Palliative Care Services with in the service line.

Qualifications

EDUCATION

Completion of ACGME accredited residency program. Board Certification in Hospice and Palliative Medicine or Pain Medicine

EXPERIENCE

Must have experience in the care of patients with advanced illness and pain

PHYSICAL DEMAND

MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged.
