Geisinger, a national leader in healthcare innovation and technology, is seeking a BC/BE pallative care physician for an exciting opportunity with the Geisinger at Home team, based in Danville, PA.  Geisinger at Home is a portfolio of services designed to provide integrated clinical care that manages patients with multi-morbid medical conditions primarily within their homes by delivering Home Based Medical Care (HBMC). The program is designed to deliver a spectrum of clinical services that help high-cost, complex patients with care at home to avoid tertiary settings when possible.

 

Home Based Medical Care team includes:

Advanced Practitioner Direct Care Case Manager Direct Care Team Community Health Assistants Field Coordinator Physicians   JOB SUMMARY:

Functions as a member of the multidisciplinary treatment team for Geisinger at Home, specifically within Community Based Palliative Care. Will deliver care to patients directly in their home who are part of the Geisinger at Home program in the Central Region of Pennsylvania. The patients include those with six or more chronic who are high utilizers of care. These patients will require symptom management and advanced care planning discussions. Will provide oversight and review the work of the Palliative Care CRNPs and RNs. Will report directly to the Chief Medical Officer of Geisinger at Home as well as indirectly to Medical Director of Palliative Care. This is a part-time position.

 

Primary job foci include:

 

1. Assessment, diagnosis and treatment Geisinger at Home patients who require palliative care.  Provides direct patient care in patients’ homes as needed and/or directly supervises care provided by the Palliative Care CRNP’s and RN’s.  This will include participation in treatment planning and referral to additional services as indicated.

 

2. Weekly interdisciplinary meetings.  This will include the Palliative Care CRNP’s, RN’s and the extended care team for Geisinger at Home. Will review patients treatment plans and goals of care.  

 

3. Functions as key component of Geisinger at Home Multidisciplinary Team

Attends/participates in Team meetings, encourages communication between team members and encourages participation of the patient and family in their care. Collaborates with internal and external team members.

 

EDUCATION AND/OR EXPERIENCE:

Physician experienced in Hospice or Palliative care. Leadership experience preferred. Physician must be working toward Palliative Care masters degree.

 

WORKING CONDITIONS AND PHYSICAL DEMANDS:

Primary work setting is in patients’ homes. Position will take call.
