§ Full spectrum of CKD care including anemia management, educational programs, and ongoing follow-up to manage transitions to dialysis modalities
§ Dialysis units and offices in multiple communities throughout Lake, Porter, and LaPorte counties – bordering the south end of Lake Michigan between the Illinois and Michigan state lines
§ Home dialysis programs with PD and HHD
§ Four new dialysis facilities opened in 2016 in partnership with American Renal Associates – one opened in 2017 and two are scheduled to open in 2018
§ Inpatient services at all main hospitals in the three-county area
