PHYSICIAN - PALLIATIVE CARE

Good Shepherd Hospice is a member of Catholic Health Services of Long Island, an integrated health care delivery system with some of the region's finest health and human services agencies. CHS includes six hospitals, three skilled nursing facilities, a regional home nursing service, hospice, and a multiservice, community-based agency for persons with special needs. CHS serves hundreds of thousands of Long Islanders each year, providing care that extends from the beginning of life to helping people live their final years in comfort, grace, and dignity.

In this role, you will work collaboratively with the Medical Director and other palliative medicine physicians and be responsible for the medical care of patients receiving Palliative Care services at St. Joseph Hospital and other CHS facilities.

This position requires local travel up to 50% throughout the Long Island area to other CHSLI facilities. Position will be on site at St. Joseph’s Hospital in Bethpage when not traveling.

Responsibilities will include:

  • Providing consultation as part of the Palliative Care interdisciplinary team in the inpatient/outpatient setting as assigned
  • Facilitating medical decision making and goals of care with patients and families
  • Establishing a written plan of care for palliative care patients in conjunction with other Palliative Care team members and the patient’s attending physician
  • Participating in weekly Palliative Care Interdisciplinary team meetings
  • Facilitating access to appropriate supportive care services for palliative care patients

The successful candidate will be a graduate from an accredited school of medicine or possess ECFMG Certification. BC/BE in Hospice and Palliative Care or Hospice Medical Director Certification and at least two  years of postgraduate direct patient care is required. Commitment to the hospice and palliative medicine principles of care for patients with serious illnesses along with the knowledge and skills in patient assessment, pain and symptom control, psychological aspects of terminal illness, pharmacotherapeutics, and up-to-date Infection Control course will be expected. Board certified in primary medical specialty, internal medicine, or family medicine, and Advanced Certification in Hospice and Palliative Medicine preferred.
