The Geriatric physician provides health assessment and direct care in all our practice environments to patients as part of the provider team. The Geriatrician provides comprehensive medical services for patients, regardless of age or sex on a continuing basis by performing the following duties outlined in this document.

Responsibilities
  • 1.Performs other related duties as follows:
  • 1. Respond promptly and with caring actions to patients and employees.
  • 2. Communicates with attending physician, ED physician, Hospitalist, Primary Care Physician regarding patient's course and further treatment.
  • 3. Communicates with attending physician in an appropriate time frame as determined by feedback from physician staff.
  • 4. Acknowledge psychosocial, spiritual and cultural beliefs and honor these beliefs.
  • 5. Maintain professional working relationships with all levels of staff, clients and the public.
  • 6. Be part of a team and cooperate in accomplishing department goals and objectives.
  • 2.Completes and submits billing and data reports in a timely fashion:
  • 1. Ensures billing codes accurately represent care provided.
  • 2. Ensures documentation/dictation supports billing codes utilized.
  • 3. Submits billing and appropriate data reports in a timely fashion.
  • 3.Demonstrates excellence in the completion of patient history and physical exams and assessments appropriate to patient population.
  • 1. Records results of lab/diagnostic work daily as appropriate.
  • 2. Demonstrates knowledge of disease stages/cognitive functioning in old age and applies development theories/ concepts when planning and implementing care for patients as indicated by feedback from staff.
  • 3. Considers factors related to safety, cost and effectiveness when planning and delivering patient care.
  • 4. Demonstrates proficiency in dictation of the history and physical and other notes on patient status.
  • 4.Quality Management:
  • 1. Contribute to the success of the organization by participating in quality improvement
  • 2. Activities
  • 3. Provides accurate medication reconciliation with each visit
  • 4. Addresses goals of care on patients yearly and with change in condition
  • 5. Ensures timely sign-outs to other members of the team
  • 6. Accepts and incorporates feedback into clinical practice
  • 5.Conducts rounds or visits on selected patients and writes note and writes progress notes.
  • 1. Communicates with the attending physicians and office staff as needed.
  • 2. Consults other services when appropriate.
  • 3. Conducts patient and family education on visit/procedure and documentation of education if applicable.
  • 4. Coordinates follow-up visits and necessary discharge care.
  • 5. Demonstrates ability to establish priorities, organize work, and complete responsibilities in a timely fashion
  • 6. Keep informed of organizational activities and promote mission and goals. Regularly check email, mail, voice mail, and other forms of communication.
  • 6.Safety/ Compliance:
  • 1. Maintain current knowledge of policies and procedures as they relate to safe work practices.
  • 2. Follow all safety procedures and report unsafe conditions.
  • 3. Follow all infection control procedures including blood-borne pathogen protocols
  • 4. Maintain privacy of all patient, employee and volunteer information and access such
  • 5. information only on a need to know basis for business purposes.
  • 6. Comply with all regulations regarding corporate integrity and security obligations.
  • 7. Report all behavior and/or activity that are unethical, fraudulent, or unlawful.
  • 8.Performs other related duties as follows. -Resident teaching with timely completion of evaluations -fellow teaching with timely completion of evaluations -presents at state, regional, national meetings:(optional)participates in volunteer work-(optional)PARTICIAPTES IN community education:-(optional )participates in peer reviewed publications:

PHYSICAL DEMAND

MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged.
