STATUS:  Full Time, Part-time    

LOCATION:  Various locations in Rochester, NY          

DEPARTMENT: Primary Care Institute


Several practice settings to align with your geriatric interest from PACE/ElderOne, Skilled Nursing Facilities, Consultative Services to Primary Care Practice


  • Evidence-based and nationally recognized care
  • Focus on maintaining seniors in the community with a goal for independent living
  • Interdisciplinary team-based care with consultative/collaborative approach with specialists
  • Provide care in a variety of settings, including PACE centers

Consultative Service

  • Partner with primary care physicians as a geriatric specialist
  • Provide approximately 90-minute consultations and 45-minute follow-up visits
  • Provide service in outpatient setting splitting time between both
  • Telephone call coverage only - average no more than two calls per week

Skilled Nursing Facility

  • Board-certified geriatricians, internal medicine or family medicine physicians
  • Desire to work with diversified patients in an acute care setting
  • Work in collaboration with nurse practitioners and physician assistants
  • Flexible work schedule with night/weekend telephone-based call coverage

Primary Care Practice Model

  • Outpatient plus hospital, nursing home and assisted living settings  
  • Dedicated panel of patients
