As a Physician on this team, you will travel to the patients’ homes. In some cases, you will travel to Skilled Nursing Facilities or Long-Term Care, in other cases Active Retirement Communities, and in some cases independent homes.

It takes a highly-skilled Physician who is confident in his or her ability to make the right decision in a crucial moment. You must feel comfortable making decisions autonomously while in the home, but must also know when the patient truly does need additional care that cannot be provided in the home setting.

Responsibilities
As a Physician your duties will be:
Assess patients’ medical, social / psychological, and financial needs regarding their healthcare
Provide same-day clinical assessment and treatment for pre-crisis intervention
Prescribe appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions
Consult with other physicians following notification of member transfer
Collaborate with UM staff and PCPs / facility staff to enable medical necessity determinations for requested medical services
- Educate members and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness
Desired Skills
Board Certified / Board Eligible Physician
Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited educational institution
Current State License
Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registrations for the purpose of writing prescriptions
- Experienced and skilled at dealing with a complex set of patients with a relatively high level of acuity
