Palliative Care Physician, Full-Time Position

Hospital-employed full-time opportunity is now available for Board Certified Palliative Care Physician at Ellis Medicine, an advanced Capital Region community hospital.

The Palliative Physician will work building a Palliative Care specialty as part of Ellis Medical Group (EMG) Primary Care, to help provide a full array of services to meet the needs of the community patients. EMG is a multispecialty physician group comprised of nearly 140 physicians and 70 mid-level practitioners. It is owned and operated by Ellis Medicine, which provides acute hospital care to the Capital Region and the Mohawk Valley of Upstate New York.

Position Highlights:

• Serves as Palliative Care Physician Champion and liaises with other Physician and Provider Champions in the other health care systems collaborating with Ellis Medicine.
• Provides palliative care consultations to EMG patients based on referrals from practice providers
• Advises Primary Care offices on palliative care work flows, use of integrated Palliative Care assessment tool and triggers for identification of palliative care patients.
• Represents DSRIP Project of integrating Palliative Care into patient center medical homes to EMG physicians and office personnel.
• Makes home visits to homebound palliative care patients, in conjunction with the Care Choices palliative home care program.
• Organizes minimum curriculum and training opportunities for Primary Care providers, including CME and use of Center to Advance Palliative Care Website.
• Shares in physician on call rotation.
• Preferred: certification in Palliative Medicine and two years of practice hospice or palliative care environment.
