I.    A.  PRIMARY PURPOSE OF AppHealthCare:

The mission of Appalachian District Health Department (AppHealthCare) is to promote and contribute to highest  level of health possible for the people of our district by:

        B.  PRIMARY PURPOSE OF POSITION:

This position, in the mountains of North Carolina, serves as Medical Director/Chief Medical Officer and part-time provider for Appalachian District Health Department and the in-scope services of the health department federally qualified health center programs. The Medical Director is the clinical leader of the interdisciplinary health team. Position provides physician medical services in the fields of family medicine, women’s health, obstetrics and pediatrics. The position may work collaboratively with other physicians for specialty services. Work includes supervision and review of work performed by mid-level providers in day-to-day operations and ensures all medical providers meet required maintenance of certification and that mid-level providers comply with the rules and regulations governing their practice standards. The Medical Director works closely with the clinical leadership team to develop and implement integrated behavioral health and care management using the driving principles of a patient centered medical home. This position also provides leadership to clinical quality improvement.

      C.  WORK SCHEDULE:

Monday through Friday 8:00 a.m. to 4:45 p.m. with 45 minute lunch with occasional after-hour workshops, meetings or health events as needed; other hours deemed necessary by Health Director. 

II.  A.  DESCRIPTION OF RESPONSIBILITIES AND DUTIES:

      Approximately 60 % Clinical Services

  • Provides medical management for all clinical services, including public health programs and those in-scope for the Federally Qualified Health Center (FQHC).
  • Provide comprehensive history, physical examinations, treatment, and related follow-up services for urgent and non-urgent presentations - addressing needs of clients across the lifespan. This includes collaborative care with tertiary providers, nutritional support, mental health providers, social agencies, school health, and others
  • Provides direct patient care, consultation, and supervision of all clinical programs, including but not limited to: Maternal Health, including High Risk maternity, Family Planning, Breast and Cervical Cancer Control, Child Health: Primary Care and preventive health (Alleghany and Ashe Counties only), Immunizations and Adult Health Primary Care (Alleghany and Ashe Counties only) and Ashe School Based Health Center.   
  • Physician supervises care provided by physician extenders. Schedule is coordinated between Watauga, Alleghany, and Ashe counties
  • Supervise and collaborate with Director of Clinical Services and with Clinical Managers as needed for annual performance appraisals of physician extenders, providing clinical leadership throughout the year as needed. 
  • Select and order tests and procedures to augment findings and/or diagnose and treat conditions
  • Diagnose client problems, including management of acute and chronic conditions
  • Perform diagnostic and/or therapeutic procedures
  • Refer clients for specialty and/or tertiary care
  • Supervise/perform diagnostic procedures, interpret data and diagnostic results, offering health education and recommendations
  • Assist/refer clients to appropriate community resources. Review and complete patient assistance forms as needed for prescription medications
  • Physician reviews and signs standing orders for maternal health, family planning, child health, sexually transmitted disease, and immunization programs 

Approximately 40% Clinical Quality Improvement/Assurance/Risk Management and other non-clinical duties

  • Works closely with the Deputy Health Director/COO and the Director of Clinical Services as a member of a collaborative leadership team to continuously improve clinical services. CMO is primarily responsible for clinical guidance, protocols, and standing orders while the Deputy Health Director/COO and the Director of Clinical Services are primarily responsible for clinical operational guidance.
  • Provides leadership to the clinical quality improvement committee, establishing clinical outcomes goals and planning, implementing and evaluating interventions.
    • Work with the Director of Clinical Services and the Quality Coordinator to create, implement, and monitor the Clinical Quality Improvement Plan. This will include developing clinical and operational goals and evaluating outcomes.  Report this work at least quarterly to the Partners in Public Health co-applicant FQHC Board.
  • Provides leadership to clinical risk management, including provider credentialing, by
    • Working with the Director of Clinical Services to coordinate and lead regular provider meetings.
    • Provide random chart audits of physician extender records to evaluate for timeliness, quality, completeness and accuracy and provide follow-up as needed.
    • Work with the Deputy Health Director/COO and the Human Resources Officer to provide oversight of the recruitment, credentialing, privileging and peer review process.
  • Work includes supervisory and administrative duties such as: serve as consultant to the medical staff, precept students from colleges/universities, and nurses who are studying to obtain higher level training, such as expanded role in adult, child and women’s health, supervises lower-level extenders, formulates protocols, policies and procedures and serves on agency committees.
  • Develops, prepares and maintains individualized patient care medical records with accuracy and timeliness, according to policies. Submits accurate documentation within 48 hours of visit.
  • Advance personal and professional growth by attending workshops and maintaining hours required for licensure. Attend agency staff meetings. Observe bylaws, policies, and procedures of DHD, NC Board of Health, NC Board of Medicine, and DEA regulations for scope of practice, participation with current membership of two or more professional organizations.
  • Participates in training required for various clinical programs such at Tuberculosis and Family Planning.
  • Other duties as assigned.

1%      Preparedness

  • Available to respond as needed to any type of public health preparedness event which may include bioterrorism, SNS, natural disaster, or other outbreak incident or event which requires the assistance and resources of the Appalachian District Health Department to protect the health and safety of the public. Staff is expected to be on “24-hour call” as needed for this type of response and to report to work as directed by the Clinical/Environmental Health Supervisor or Health Director.

B.  OTHER POSITION CHARACTERISTICS:                    

1.   Variety and Scope - Patients present with a wider variety of conditions requiring that the employee have a broader and in-depth knowledge of the area of assignment; work may be performed in a specialized area(s), such as family planning or geriatrics, or in a generalized area that requires the employee to function in a wide variety of settings and work assignments. Employees at this level frequently serve as consultants to the medical staff, participate as in-service education leaders, and may serve as a student preceptor. Work may include participating in the evaluation, development, and implementation of agency policies, procedures, goals, and objectives, and other administration functions.

2.   Intricacy - A broader variety of patient conditions present with greater complications. Employees have the ability and delegated authority from the supervising physician to make more independent and varied decisions affecting the patient's treatment plan and total care; the work environment frequently dictates the need for this role. Employees must interpret the accepted patient care protocols and medical guidelines to cover this variety and determine the appropriate course of action.

3.   Subject Matter Complexity - The employee must have a complete understanding of the patient care protocols and medical guidelines used in the area of work, and a broader understanding of the health field.

4.   Scope and Consequence of Decisions - Work has a direct effect on the patient population served. The consequence of this employee's decisions has significant impact on the patient's safety and well-being.

6.   Nature of Instructions - Daily work is self-planned and performed with considerable independence. Short-range plans and changes can usually be implemented; significant or long-range plans and changes are coordinated with the members of the health care team (i.e., pharmacist, physical therapist, nutritionist, etc.).

7.   Nature of Review – The CMO works independently within scope of medical licensure. In addition, the ADHD provider team completes a peer review at least annually which is reported through the Clinical QI/QA Committee. 

8.   Scope of Contacts - Work requires contacts with specific patient population and families who may be unfamiliar with the work function, or with special groups as mental patients or prisoners; may also have contact with physicians, nurses, and other medical personnel.

9.   Nature and Purpose - Contacts frequently require persuading and influencing patients to pursue medical treatment or another course of action.                    

10. Hazards - Periodically exposed to patients who may be difficult to work with and a mildly disagreeable work environment.

III.  KNOWLEDGE, SKILLS, & ABILITIES AND EXPERIENCE REQUIRED:

A.  Knowledge, Skills, and Abilities

Thorough knowledge of the theory, principles, and practices of general, preventive medicine and of the techniques involved; thorough knowledge of recent developments in the field of medicine; thorough knowledge of the practice of the medical specialty or sub-specialty of which assigned.

Skill in the treating and diagnosing of a variety of diseases and injuries and in the practice of an applicable specialty of medicine; ability to compile clear and concise notes for patients' charts and to make oral presentations of cases in medical conferences; ability to guide work performance of physicians with limited experience; ability to establish and maintain effective working relationships with associate personnel, patients, and their families, and the general public.

B.  Required Minimum Training

Graduation from an accredited school of medicine and completion of residency or specialty training. Prefer at least one year of experience in Public Health, Women’s Health, or specialty area of medicine or graduation from an accredited school of medicine and two years of experience in the practice of medicine or an equivalent combination of education and experience.
