Geisinger Medical Center (GMC), located Danville, Pa., the largest tertiary/quaternary care teaching hospital in the region, is licensed for 475 beds, including 89 pediatric beds in the Janet Weis Children’s Hospital.  GMC, a MagnetHospital, maintains the region’s only Level 1 Regional Resource Trauma Center with additional qualifications in pediatrics. GMC offers an array of highly specialized medical and surgical services, including neurosciences, cardiovascular services, transplantation, women’s health (in the Women’s Pavilion), oncology and orthopaedics.  The Hospital for Advanced Medicine serves as an integrated center for the most critical patients. Outpatient services are available on and off campus.

The Medical Director, Health and Wellness under the oversight of the Chief Population Health Officer, is jointly responsible for Health Plan / Population Health activities related to health, wellness and the social determinants of health in conjunction with the AVP, Health and Wellness. He or she will be dedicated to the goals and objectives of optimizing health and wellness across Geisinger’s community footprint and will act as a visible champion supporting strategies that support the health of those served by Geisinger and the communities we serve.

Performs duties to support the goals and objectives of the Geisinger Health System in its efforts to provide quality patient/member care, community health and an exceptional patient/member  experience.

Works directly with the AVP Health and Wellness and the Chief Population Health Officer with a matrixed supervisory relationship to the Chief Medical Officer of Geisinger Health Plan.


MAJOR DUTIES AND RESPONSIBILITIES:
Social Determinants of Health:
- Functions as the physician leader in the collaboration and development of strategies related to the social determinants of health along with other key team members aligned with this work.
- Takes a leadership role to evaluate community needs assessments and other indicators available to establish key priorities for Geisinger’s activities related to the social determinants of health.
- Responsible for the continued development and implementation of the GHP Health and Wellness Plan for all members and Geisinger employees in conjunction with the AVP Health and Wellness.
- Works closely with GHP Regional Medical Directors and hospital campus leaders to identify and monitor key performance metrics to evaluate the effectiveness of programs.
- Provides clinical leadership in the development and implementation of new community based programs targeted toward the social determinants of health.
- Serves as an ambassador to the community to engage appropriate stakeholders.
- Assists with fundraising to support community based programs.

Health and Wellness:
- Provides physician oversight for the Health Plan’s health and wellness management programs.
- Evaluates trends in chronic disease and in collaboration with the AVP Health and Wellness leads the development of new programs based on assessed need and best practices.
- Provides clinical leadership in the implementation of new health and wellness programs

Administration:
- Functions as a member of the Health Plan and Population Health operations team and administrative staff.
- Participates in weekly Medical Directors meeting and Senior Leadership Population Health meetings.
- Performs other duties as required or assigned by emergency or other operational reasons for which the employee is qualified to perform.

SKILLS AND ABILITIES:
- Demonstrated ability to understand community health needs, data interpretation and implementation of population health approaches that support the Triple Aim.
- Demonstrated dedication to the philosophy of managed care and continuous quality improvement of clinical care and service required.
- Demonstrated ability to communicate effectively at all levels of a large complex organization required.


EDUCATION AND/OR EXPERIENCE:
- Current Pennsylvania licensure to practice without restrictions required.
- MD/DO degree with preference toward a primary care specialty and previous experience in population health or health and wellness programs.
- Board certification in chosen specialty required.

WORKING CONDITIONS/PHYSICAL DEMANDS:
- Work is typically performed in an office environment but will require independent travel to community and employer –based events.
