2 week assignment. At least 5 years experience as a Family Practitioner strongly desired. CO License and Current DEA license required. Must obtain and maintain credentialing privileges with facility. When first submitting candidates, include at least 2 references, a resume, and a skills checklist/competency assessment. Shifts may fluctuate to fulfill the department needs. Located 3 hours northwest of Denver in a unique small town, perfect for outdoor enthusiasts. We work and play in the most beautiful setting in the world offering many recreational activities throughout the year. This includes skiing, snowboarding, snowshoeing, camping, fishing, hiking, golfing, mountain biking and much more. Requirements for Traveler: · Proof of immunity to Rubella, Measles, Mumps, Varicella-Zoster; and Tetanus and Annual Flu Shot · Verification of a negative PPD within the past 90 days and two-step follow up PPD taken within 2nd week of first shift worked at Facility, or documentation of current chest x-ray. A second PPD is not needed if a PPD has been applied at any time within the last 90 days and the result was negative. If Agency Professional is unable to conduct/produce verification prior to the start of the assignment, the PPD can be conducted at the facility for a fee.
Documentation of Hepatitis B vaccine or annual statement of declination. · General statement of physical health; · Criminal Background Check · Sanction Screening (SAM, GSA, OIG, etc.) · Drug Screen (within 14 days of start date)
