Cleveland Clinic’s Center for Connected Care is searching for a physician to lead the clinical activities of a new medical service called “Hospital At Home” (HaH). HaH will deliver hospital-level care for acutely ill adults in their own homes, with the goal of avoiding utilization of an acute hospital. 

Patients who present to the emergency department with uncomplicated heart failure exacerbation, COPD exacerbation, asthma exacerbation, UTI, or cellulitis will be considered for the service. Patients will have to meet inclusion criteria indicating that they are sick enough to be considered for inpatient hospitalization, but are not otherwise unstable or are considered at high risk for further progression to critical illness. Diagnostic dilemmas will have to be resolved in the emergency department prior to admission to program. 

Qualified candidates for this clinical role will include internists who are comfortable caring for acutely ill adults, or emergency room physicians. The job will require a combination of house calls, virtual visits, and onboarding patients in the emergency department. Physician will have a prominent leadership role on this team of advanced practice practitioners, nurses, and other support staff from Connected Care. Physician will be involved with developing protocols and managing relationships with key partners across the Enterprise. There will be significant academic opportunities as this is an entirely novel clinical model of care. Preferred candidates for this position are those open to novel models of care, excellent communicators, possess leadership and critical thinking skills, and have a passion for innovation.
