Cleveland Clinic is seeking physicians to join the Center for Connected Care. The Center for Connected Care is a critical piece of the Cleveland Clinic's transformation into a value-oriented healthcare system. The existing opportunities available within the Center include both house calls and skilled nursing facility work in the Akron market. As part of the Cleveland Clinic, you will be connected to the 2,000+ medical group practices through a common electronic medical record.

The Akron market will be an expansion of a house calls practice that has been in existence since 2008. Training and support will be provided by these existing practices. In addition, you will enter into a call rotation with the existing practices.

House Calls Opportunity:

  • Provide primary care in homes and assisted living facilities in the Akron market.
  • Serve as the collaborating physician of existing nurse practitioners.
  • Work collaboratively with homecare teams.
  • Opportunities exist for geriatric and/or palliative medicine consultation.

Candidates must be board certified/eligible in Internal Medicine, Family Medicine or Geriatric Medicine. In addition, there will be opportunities available for: leadership responsibilities, research activities, education and clinical program development.
