SUMMARY:

Physician executive responsible for the medical leadership of the population health functions within CareMount Medical. This individual reports directly to the Chief Population Health Officer, with an indirect reporting relationship to the Chief Medical Officer.

PRIMARY RESPONSIBILITIES:

  • Defining quality standards to be achieved by all providers within the Group and/or responsible for other patients under population health management arrangements
  • Overseeing the development, installation, and monitoring of programs and systems to evaluate the quality, value, and efficiency of care across the Group
  • Developing and managing practice transformation services for primary and specialty care, specialized clinical solutions, patient engagement programs, and any other functions or services necessary for the effective management of populations
  • Developing and managing the enterprise-wide care management function, and disease and utilization management capabilities in support of the population health transformation
  • Serving as the CEO of the CareMount Medical Accountable Care Organization and in administrative capacities within the Group and/or other future physician alignment vehicles as appropriate
  • Maintaining accountability for respective operational and financial performance of population health clinical programs and services
  • Providing professional leadership and direction to the clinical staff, quality improvement, and care management/coordination teams
  • Analyzes current QA performance to identify opportunities for improvement, oversees development supporting measurement and reporting strategies
  • Oversee and provide strategic direction for the development, implementation, training, and maintenance of databases and reports supporting on-going performance improvement
  • Partner with medical staff leaders to align performance indicators to support pay for performance initiatives, peer review, physician performance expectations, and outcomes studies
  • Develop and execute data strategies to support implementation of innovative care delivery models aligned with the Triple Aim and value­ based care in the inpatient and outpatient setting
  • Define qualitative and quantitative data reporting needs with senior management. Provides clinical oversight for the development of performance reports. Reviews and validates newly develo'ped reports to ensure accuracy and effectiveness
  • Manage the reliable, complete, and consistent flow of patient information, including the collection and reporting of clinical indicators and measures and support of clinical decision support functions

KNOWLEDGE AND SKILL REQUIREMENTS:
1. Medical Degree in a primary care specialty
2. 10+ Years of Quality Programs oversight

WORKING CONDITIONS:
Working conditions are normal for an office environment. Work may require occasional weekend and/or evening work.
