Harbor Health Service’s PACE (Program of All-inclusive Care for Elderly) is seeking a Medical Director for our Boston based program. This program, The Elder Service Plan, is an innovative, comprehensive care management model serving frail elders aged 55 and above. The Elder Service Plan is one of 116 PACE programs in America.

Harbor Health’s PACE program is expanding to a second site in the spring of 2017 and seeks Medical Director level support to manage our current staff of three geriatricians, one psychiatrist, and three geriatric nurse practitioners as well as MD/NP staff that will join us at our second location. As a physician leader in this program, the Medical Director will be responsible for recruiting, supervision, and evaluation of these clinicians and will also have a caseload of patients.

Key elements of PACE model of care and Medical Director’s role in program:

  • 11-Member Interdisciplinary Team manages the care of our frail elders and supports each other and the participant in achieving care goals.
  • Participants attend our center for Adult Day Health activities as well as receive on-site clinical care including primary care, behavioral health, and rehabilitation.
  • Our physicians follow our elders throughout all locations of care including acute, sub-acute, long-term, and home care settings. 
  • Small Patient panel (approximately 150 patients per Clinical Team).
  • Regular home visits to our participants are encouraged and time is allotted for this effort.
  • PACE is a completely capitated program which allows us the flexibility and creativity to provide the best, individualized care for our patients.
  • Quality Improvement efforts are a key element of our care model. The Medical Director will lead these initiatives.

We are seeking an IM or Family MD with completion of a Geriatric Fellowship program or a BC/BE Geriatrician, five-plus years of experience in Internal Med or Geriatrics, minimum of three years of leadership or administrative experience.
